Pewnego dnia psychiatra - Malcolm Crowe - zostaje postrzelony przez jednego ze swoich pacjentów, który następnie odbiera sobie życie. Od tej pory bohater ma problemy w domu. Żona zachowuje się jakby go nie było, a on sam nie potrafi do niej dotrzeć. W końcu lekarz sam zaczyna popadać w depresję. Badając ośmioletniego chłopca Cole'a Seara psychiatra dostrzega, że chłopiec posiada szósty zmysł. Cole widzi i słyszy umarłych. Lekarz postanawia mu pomóc.
